Labler for cable tags

Does anyone know of a labler that will produce a tag 4 inches in height to hold 12 lines at size 12 font. I need it to lable fiber cables 144 strand and the counts are broken out and they want it all on one tag.

You could try to type it out on the computer, print it and lay the paper under the laminate on your tag (assuming you have that type). Adasm Sickel an I tried it and it fits. The 12 lines measure 2.75" Top to Bottom. Good luck.
